Policy Statement
In accordance with the requirements of Title II of the Americans with Disabilities Act of 1990 (ADA) and Section 504 of the Rehabilitation Act of 1973, Seeds of Health does not discriminate on the basis of disability in its services, programs, activities, or employment practices.

Requesting Accommodations
Grievance Procedure
This grievance procedure is established to meet the requirements of Title II of the ADA (28 C.F.R. § 35.107(b)) and Section 504 (34 C.F.R. § 104.7(b)). It may be used by anyone who wishes to file a complaint alleging discrimination on the basis of disability in the provision of services, activities, programs, or benefits by Seeds of Health.
Complaints must be submitted in writing and include the name, address, and telephone number of the complainant, along with a description of the alleged discriminatory act. The description should include the location, date, and, if known, the names of any individuals involved. The complaint should be submitted by the grievant or their designee as soon as possible, but no later than sixty (60) calendar days after the alleged violation, to:

Within fifteen (15) business days of receiving the complaint, the Chief Human Resources Officer or their designee will begin an investigation, which may include contacting the complainant for additional information. A written response will be issued within thirty (30) calendar days of receipt of the complaint, where feasible. The response will explain the Agency’s position and offer options for resolution. If the complainant is not satisfied with the response, they may appeal the decision within fifteen (15) calendar days to the Chief Executive Officer, who will issue a final determination within thirty (30) days of receiving the appeal.
