If you would like to mail your gift to Seeds of Health, please print off and complete the form below and mail it with your check or money order to:
Matching Gift
Check with your employer's human resources department to see if your donation is eligible for a Matching Gift. Your donation could be eligible for double, or sometimes triple, the amount. Please send the appropriate employer matching gift paperwork with your gift.
Memorial or Tribute
Consider making a memorial or tribute donation in honor of someone special. Provide the name of the person/s for whom the gift is being made along with their contact information, if appropriate. We will gratefully acknowledge your gift without disclosing the amount.
